Psychologen definieren hier schon den Anfang einer <strong>Essst\u00f6rung<\/strong>. Die Erkrankung f\u00fchrt neben den k\u00f6rperlichen Auswirkungen auch zu sozialen Problemen und psychischen St\u00f6rungen, da die Auswirkungen f\u00fcr das Umfeld meist direkt sichtbar sind. Die Erkrankung wird in zwei Typen unterteilt, wobei dem restriktiven Typus eine Verweigerung der Nahrungsaufnahme zugrunde liegt, dem sogenannten Purgin Typus ausserdem eine erzwungene Gewichtsreduzierung durch z.B. Erbrechen, exzessiven Sport, Abf\u00fchrmittel oder Appetitz\u00fcgler. Die Ursache der Erkrankung ist ein St\u00f6rung der Wahrnehmung oder auch K\u00f6rperschemast\u00f6rung, bei der der Betroffene sich selbst als zu dick wahrnimmt, obwohl er bereits an Untergewicht leidet. Das Selbstwertgef\u00fchl der Betroffenen h\u00e4ngt nahezu ausschlie\u00dflich von der eigenen Kontrolle des K\u00f6rpergewichts ab. Leistungen in Beruf, Privatleben und eventuellen Hobbys treten als Indikator dabei stark in den Hintergrund. Die Gedankenwelt kreist vorwiegend um das Thema Ern\u00e4hrung, Figur und Gewicht und wird stark eingeschr\u00e4nkt. Obwohl kaum gegessen wird ist die Nahrung dennoch der zentrale Lebensmittelpunkt. Betroffene sind zu \u00fcber 90% Frauen, besonders wenn der Beruf ein niedriges K\u00f6rpergewicht fordert. Das Gewicht der Erkrankten ist in der Regel normal, denn die Hei\u00dfhungerattacken werden durch gegenregulatorische Ma\u00dfnahmen kompensiert. Dazu z\u00e4hlen selbstausgel\u00f6stes \u00dcbergeben, starkes Hungern, extreme Di\u00e4ten, exzessiver Sport, die Einnahme von Abf\u00fchrmitteln, oder Brechmitteln. Die Attacken sind zeitlich nicht regelm\u00e4ssig und haben Abst\u00e4nde von Stunden bis Wochen. Als Ausl\u00f6ser der Ess-Brechsucht gelten meist emotionale Ursachen, starker Stress und Einsamkeitsgef\u00fchle. Das durch das \u00dcbergeben induzierte Defizit an Nahrung f\u00fchrt ausserdem zu erneuten Essattacken. Die Nahrungsaufnahme ist w\u00e4hrend der Anf\u00e4lle unkontrolliert und ein Verlust der Selbstkontrolle ist zu beobachten. Das folgende Erbrechen wird ausgel\u00f6st von Schamgef\u00fchlen, Angst vor Gewichtszunahme und dem Gef\u00fchl des Versagens bei der Selbstkontrolle. Eine <strong>Binge-Eating Essst\u00f6rung<\/strong> oder auch Binge-Eating Disorder liegt dann vor, wenn ein Betroffener zu wiederkehrenden Hei\u00dfhungeranf\u00e4llen neigt, die mit dem Verlust der eigenen Kontrolle einhergehen. Diese &#8220;Fressanf\u00e4lle&#8221;, wie sie von Erkrankten oft genannt werden, f\u00fchren mittelfristig oft zu starker Gewichtszunahme bei vielen Betroffenen. Im Gegensatz zur Anorexia Nervosa (Purgin Typus) wird die aufgenommene Nahrung nicht erbrochen. Diagnostisch bestimmt man diese Essst\u00f6rung, wenn \u00fcber einen Zeitraum von mindestens 6 Monaten, mindestens 2 oder mehr unkontrollierte Essanf\u00e4lle auftreten, die von einem Verlust des S\u00e4ttigungsgef\u00fchls bei hoher Kalorienaufnahme gepr\u00e4gt sind. Dabei wird die Nahrungsaufnahme nicht von einem Hungergef\u00fchl ausgel\u00f6st und endet erst bei einem \u00fcberm\u00e4ssig starken V\u00f6llegef\u00fchl, w\u00e4hrend die Nahrung regelrecht verschlungen wird. Spricht man jedoch von einer <strong>latenten Esssucht<\/strong>, so ist ein geplantes und permanentes, sehr streng \u00fcberwachtes Essverhalten bei Betroffenen zu beobachten, dass einer selbstauferlegten lebenslangen Di\u00e4t gleichkommt. Die Figur der Betroffenen ist in den meisten F\u00e4llen normal, bis ideal. Im Mittelpunkt des Alltags steht das Essen und die eigene Figur, denn es herrscht die \u00dcberzeugung, dass ohne eine Dauerdi\u00e4t eine ummittelbare Gewichtszunahme resultieren w\u00fcrde. Sollte aus irgendeinem Grund einmal zuviel gegessen werden, so wird dies sofort mit einer radikalen Di\u00e4t ausgeglichen, die bis zum Verzicht auf Nahrung gehen kann. Betroffene sind auch hier vorwiegend Frauen, die meist ein gest\u00f6rtes Selbstbild und eine Fremdartigkeit gegen\u00fcber dem eigenen K\u00f6rper entwickelt haben. Der Wandel einer latenten Esssucht zu einer Bulimie oder Magersucht ist \u00fcbergangslos und schnell vollzogen.
